We are seeking a multidisciplinary healthcare professional who is licensed both as a Registered Nurse (RN) and a Physical Therapist (PT) (or closely collaborating within a hybrid care model). This unique role focuses on delivering comprehensive patient care at home, blending clinical nursing services with rehabilitative physical therapy support. Ideal for professionals passionate about holistic, in-home care, rehabilitation, and chronic disease management.
Conduct in-home health assessments, physical evaluations, and patient history reviews
Create and implement individualized care plans and therapy regimens
Provide skilled nursing services (e.g., wound care, medication management, IV therapy)
Deliver physical therapy interventions to restore mobility, function, and reduce pain
Educate patients and families on both clinical care and rehabilitative exercises
Monitor patient progress and modify treatment plans based on evolving needs
Maintain thorough and timely clinical documentation (EMR, OASIS, therapy notes)
Collaborate with primary care providers, case managers, and home care aides
Ensure safety, infection control, and compliance with Medicare, HIPAA, and state guidelines
Active Registered Nurse (RN) license
Active Physical Therapist (PT) license (or coordination experience with PTs if RN only)
2+ years of experience in home health, rehab, or community nursing
Familiarity with OASIS documentation and therapy reporting tools
CPR/BLS certification
Valid driver’s license and reliable transportation
