We are seeking a licensed and compassionate Physical Therapist (PT) to evaluate, plan, and deliver therapy services that help patients improve mobility, reduce pain, restore function, and prevent disability. The Physical Therapist will work with patients across various conditions including orthopedic, neurological, post-surgical, and chronic pain cases.
Perform patient evaluations, functional assessments, and mobility testing
Develop individualized treatment plans based on diagnosis and patient goals
Provide hands-on therapy, therapeutic exercises, and manual therapy techniques
Educate patients and caregivers on home exercise programs and injury prevention
Monitor progress, modify treatment plans, and document outcomes
Use modalities as needed (heat, cold, ultrasound, e-stim, etc.)
Coordinate care with physicians, occupational therapists, and rehab teams
Maintain accurate documentation in EMR systems and comply with regulations
Support discharge planning and ensure continuity of care
Ensure patient safety, infection control, and quality standards
Doctor of Physical Therapy (DPT) or equivalent degree
Active Physical Therapist license (state-specific)
Strong knowledge of anatomy, kinesiology, and rehabilitation protocols
Excellent communication, empathy, and clinical decision-making skills
Experience in outpatient, inpatient, home health, or rehab settings
Orthopedic, neuro, or sports therapy experience is a plus
CPR/BLS certification preferred
Familiarity with EMR documentation systems
