Raze Therapeutics is seeking a highly motivated and experienced Physical Therapist PT for a Long term-week contract position in Forks, WA (98331). As a Physical Therapist PT at Raze Therapeutics, you will be responsible for administering and directing the examination, evaluation, and determination of physical therapy diagnoses, rehabilitation prognoses, and procedural interventions for patients requiring physical therapy services.

Responsibilities:

Administer and direct physical therapy examination, evaluation, and diagnosis

Determine physical therapy rehabilitation prognosis and procedural interventions

Treat patients in an outpatient setting (80%) and in an acute/swing bed (15%) and long-term care (5%) setting

Conduct one-on-one patient treatments for approximately 45 minutes

Document patient progress and complete necessary administrative tasks

Maintain patient confidentiality and adhere to HIPAA regulations

Requirements:

Active state license in Physical Therapy in the state of Washington

Experience administering and directing physical therapy examination, evaluation, and diagnosis

Ability to work in a fast-paced environment and manage a varied patient case load

Strong communication and interpersonal skills

Schedule:

Monday to Friday from 8:00 a.m. to 5:00 p.m. with a lunch break from 12:00 p.m. to 1:00 p.m.

Schedule may vary based on patient needs and staffing availability
