Raze Therapeutics is seeking an experienced Occupational Therapist to join our team in Thief River Falls, MN. The therapist will be responsible for providing therapy services primarily in SNF and HH settings, with some outpatient services. The ideal candidate will have a current active state license and be available to work Monday to Friday from 8:00 am to 4:30 pm.
Responsibilities:
Evaluate patients and create individualized treatment plans
Administer therapy sessions, including hands-on treatment, modalities, and exercises
Collaborate with other healthcare professionals to ensure the best patient outcome
Document patient progress and adjust treatment plans as needed
Requirements:
Active state license in Occupational Therapy
Previous experience in SNF and HH settings preferred
Excellent interpersonal and communication skills
Ability to work independently and as part of a team
