Title:  Physical Therapist - Mauston,Wisconsin
Job Summary:
We are seeking a licensed Physical Therapist to join our team in Mauston, WI. As a Physical Therapist, you will be responsible for developing and implementing individualized physical therapy programs to help patients achieve functional goals. You will work closely with other healthcare professionals and ancillary staff to provide excellent care to patients.

Job Requirements:

Bachelor's degree in Physical Therapy from an accredited program by the American Physical Therapy Association (APTA} Commission on Accreditation in Physical Therapy Education
Current Physical Therapist license to practice in the state of Wisconsin
Ability to care for the physical and developmental needs of the population age groups that will be receiving services
Ability to remain calm in emergency or crisis situations
Extensive standing and walking, lifting, positioning, pushing, and/or transferring patients
Frequent reaching, stooping, bending, kneeling, and crouching
Exposure to infectious diseases and human waste products
Essential Functions:

Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists as needed
Reviews patient's functional needs and adjusts care plan as indicated to ensure optimal patient outcome
Designs and implements a plan of care for the patient based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate
Conducts therapy practice within defined standards of care
Directs the assistance of support staff in order to achieve the patient's functional goals
Communicates effectively with other professional and support staff in order to achieve positive patient outcomes
Practices established hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards
Documents evaluations, treatment goals, and plans using appropriate electronic medical software, in a timely fashion. Regularly updates documentation and maintains all required prescriptions and signatures.
Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on current professional literature.
Provides direction and leadership to staff and acts as a resource to promote physical therapy within the community

Demonstrates excellent customer service through attitude and actions, consistent with the mission, values and standards of performance
Adheres to AIDET communication framework in every encounter, both in person and over the phone
Follows the AIDET 10/5 rule by making eye contact and smiling within 10 feet of someone, and speaking and acknowledging within 5 feet
Manages up co-workers and other departments within the organization
Provides a positive and professional representation of the organization
