We are seeking a skilled Physical Therapist to join our team and provide compassionate care to our patients in their homes. The ideal candidate will have a current active state license, previous experience in home health, and be vaccinated.

Responsibilities:

Conduct patient assessments and develop treatment plans
Provide skilled physical therapy services to patients in their homes
Communicate with healthcare team members regarding patient progress
Educate patients and their families on proper home exercise programs and activities of daily living
Complete accurate documentation in a timely manner
Requirements:

Active state license as a Physical Therapist
Previous experience in home health
Vaccinated
Ability to work independently and as a part of a team
Strong communication and interpersonal skills
Reliable transportation
