Job Title: Physical Therapist / Physical Therapist Assistant - Long Term Care (LTC)
Job Summary:
We are seeking a dedicated and compassionate Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our team in a Long Term Care facility. The ideal candidate will provide high-quality rehabilitation services to residents to help improve their mobility, strength, and overall quality of life.
Responsibilities and Duties:
- Evaluate residents' physical conditions and develop personalized treatment plans
- Implement and administer therapeutic exercises and modalities
- Work collaboratively with other healthcare professionals to ensure comprehensive care for residents
- Monitor and document progress and adjust treatment plans as needed
- Educate residents and their families on therapeutic exercises and home exercise programs
Qualifications:
- PT: Doctorate in Physical Therapy from an accredited program; licensure in the state of practice
- PTA: Associate's degree in Physical Therapy Assistant; licensure in the state of practice
- Prior experience in Long Term Care or geriatric rehabilitation preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
