Job Title: Physical Therapist (PT) - Long Term Care (LTC)
Job Summary:
We are seeking a dedicated and compassionate Physical Therapist to join our Long Term Care facility. The Physical Therapist will be responsible for developing individualized treatment plans for patients, conducting therapy sessions, monitoring progress, and educating patients and their families on proper techniques and exercises. The ideal candidate will have a strong background in geriatric care and a passion for improving the quality of life for elderly patients.
Responsibilities and Duties:
- Develop and implement individualized treatment plans for patients
- Conduct therapy sessions to improve mobility, strength, flexibility, and overall function
- Monitor progress and adjust treatment plans as needed
- Educate patients and their families on proper techniques and exercises
- Collaborate with other healthcare professionals to provide comprehensive care
- Maintain accurate and up-to-date patient records
- Stay current on best practices and advancements in physical therapy
Qualifications and Skills:
- Doctorate or Master's degree in Physical Therapy
- Valid state license to practice as a Physical Therapist
- Previous experience working in Long Term Care facilities preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient-centered approach to care
- CPR certification
