Job Title: Physical Therapist - Long Term Care (LTC)
Job Summary:
As a Physical Therapist in a Long Term Care facility, you will be responsible for providing comprehensive physical therapy services to residents with chronic conditions and disabilities. You will work closely with interdisciplinary teams to develop individualized treatment plans and ensure the highest level of care for each resident.
Responsibilities and Duties:
1. Evaluate and assess residents' functional abilities and limitations
2. Develop individualized treatment plans to improve mobility, strength, and coordination
3. Implement therapy interventions to help residents achieve their goals
4. Educate residents and their families on therapeutic exercises and activities
5. Collaborate with healthcare professionals to coordinate care and optimize outcomes
6. Monitor progress and adjust treatment plans as needed
7. Document clinical findings and treatment outcomes accurately
8. Participate in interdisciplinary team meetings to discuss resident progress and goals
Qualifications and Skills:
1. Doctorate or Master's degree in Physical Therapy
2. Valid state license to practice as a Physical Therapist
3. Experience working in Long Term Care settings preferred
4. Strong communication and interpersonal skills
5. Ability to work effectively in a team environment
6. Compassionate and patient-centered approach to care
