Job Title: Occupational Therapist (OT) - Long Term Care (LTC)
Job Summary:
We are seeking a skilled and compassionate Occupational Therapist (OT) to join our Long Term Care (LTC) facility. The ideal candidate will help improve the quality of life for our residents by promoting independence and enhancing their ability to perform daily activities.
Responsibilities and Duties:
1. Evaluate residents' physical and cognitive abilities to determine individualized treatment plans.
2. Provide therapeutic interventions to improve residents' strength, endurance, and functional ability.
3. Develop and implement occupational therapy programs to enhance residents' daily living skills.
4. Collaborate with interdisciplinary team members to ensure comprehensive care for residents.
5. Educate residents and their families on adaptive equipment, home modifications, and community resources.
6. Document residents' progress and communicate information effectively to team members.
7. Participate in discharge planning and coordinate care transitions for residents.
Qualifications and Skills:
1. Valid Occupational Therapist (OT) license in the state of practice.
2. Graduation from an accredited Occupational Therapy program.
3. Prior experience working in a Long Term Care (LTC) setting is preferred.
4. Strong communication and interpersonal skills.
5. Ability to work independently and collaboratively within a team.
6. Proficiency in computer skills for documentation purposes.
7. Compassionate and patient-centered approach to care.
