Job Title: Occupational Therapist (OT) - Long Term Care (LTC)
Job Summary:
We are seeking a dedicated Occupational Therapist to join our Long Term Care facility and provide high-quality therapy services to residents. The Occupational Therapist will work with individuals who have disabilities, injuries, or illnesses to help them develop, recover, and improve the skills needed for daily living and working. The OT will create individualized treatment plans, conduct evaluations, and collaborate with other healthcare professionals to ensure the best possible outcomes for our residents.
Responsibilities and Duties:
1. Conduct assessments to identify residents' strengths and limitations
2. Develop personalized treatment plans to improve residents' functional abilities
3. Implement therapeutic activities to enhance residents' mobility, strength, and independence
4. Collaborate with the interdisciplinary team to coordinate care and monitor progress
5. Provide educational resources and support to residents and their families
6. Document all services provided in a timely and accurate manner
7. Stay current on best practices and evidence-based interventions in occupational therapy
8. Participate in quality improvement initiatives to enhance resident outcomes
Qualifications and Skills:
- Valid Occupational Therapy license in the state of practice
- Master's degree in Occupational Therapy
- Experience working in Long Term Care settings preferred
- Strong communication and interpersonal skills
- Ability to work independently as well as part of a team
- Proficiency in electronic medical records systems
- CPR certification
- Compassion and empathy for residents' diverse needs
