Job Title: Occupational Therapist (OT) - Long Term Care (LTC)
Job Summary:
As an Occupational Therapist (OT) specializing in Long Term Care (LTC), you will work with elderly and disabled patients to improve their ability to perform daily activities and maintain independence. Your role will involve assessing patients' physical and mental capabilities, developing individualized treatment plans, and implementing therapeutic interventions to enhance functionality and quality of life.
Responsibilities and Duties:
1. Conduct comprehensive evaluations to determine patients' physical, cognitive, and psychosocial needs.
2. Develop personalized treatment plans based on evaluation findings and patients' goals.
3. Provide hands-on therapy sessions to improve patients' strength, coordination, balance, and mobility.
4. Collaborate with interdisciplinary team members to ensure coordinated care and effective communication.
5. Educate patients, families, and caregivers on adaptive techniques and equipment to promote independence.
6. Monitor progress and adjust treatment plans as needed to achieve optimal outcomes.
7. Document patient assessments, treatment interventions, and progress notes in accordance with regulatory standards.
8. Participate in discharge planning and recommend appropriate community resources for ongoing support.
Qualifications and Skills:
1. Graduate from an accredited Occupational Therapy program.
2. Valid Occupational Therapy license in the state of practice.
3. Experience working with geriatric and long-term care populations preferred.
4. Strong clinical reasoning and problem-solving skills.
5. Excellent communication and interpersonal abilities.
6. Ability to work independently and collaborate effectively with team members.
7. Proficiency in using therapy modalities and adaptive devices.
8. Commitment to providing compassionate and patient-centered care.
