Job Title: Speech and Language Pathologist (SLP) - Long Term Care (LTC)
Job Summary:
We are seeking a skilled and compassionate Speech and Language Pathologist to join our Long Term Care (LTC) team. The SLP will be responsible for assessing, diagnosing, and treating speech, language, and communication disorders in elderly patients residing in long term care facilities.
Responsibilities and Duties:
- Conduct comprehensive evaluations to assess patients' speech, language, and swallowing abilities
- Develop individualized treatment plans to address communication and swallowing disorders
- Provide therapeutic interventions such as exercises, activities, and strategies to improve speech and language function
- Collaborate with other healthcare professionals, including nurses, doctors, and occupational therapists, to ensure coordinated care
- Monitor patients' progress and adjust treatment plans as needed
- Educate patients, families, and caregivers on communication strategies and exercises to improve outcomes
- Maintain accurate and timely documentation of patient care
Qualifications and Skills:
- Master's degree in Speech-Language Pathology from an accredited program
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
- Valid state licensure as a Speech-Language Pathologist
- Prior experience working with elderly patients in a long term care setting is preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Strong critical thinking and problem-solving abilities
- Proficiency in electronic medical record systems
