Job Title: Occupational Therapist (OT) Long Term Care (LTC)
Job Summary:
We are seeking a skilled and compassionate Occupational Therapist (OT) to join our Long Term Care (LTC) facility. The OT will be responsible for evaluating, planning, and implementing individualized treatment programs to help residents improve their functional abilities and overall quality of life.
Responsibilities and Duties:
- Conduct assessments to determine residents' strengths and limitations
- Develop personalized treatment plans to address residents' rehabilitation needs
- Provide therapy services, including but not limited to, therapeutic exercise, activities of daily living training, adaptive equipment training, and cognitive retraining
- Collaborate with interdisciplinary team members to ensure comprehensive care for residents
- Monitor residents' progress and adjust treatment plans as needed
- Educate residents and their families on therapeutic techniques and strategies for independent living
- Maintain accurate and timely documentation of therapy services provided
Qualifications and Skills:
- Bachelor's or Master's degree in Occupational Therapy from an accredited program
- Current state licensure as an Occupational Therapist
- Minimum of 1-2 years of experience in a Long Term Care (LTC) setting preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong clinical reasoning and problem-solving abilities
- Proficient in electronic medical records documentation
