Job Title: Physical Therapist (PT) - Long Term Care (LTC)
Job Summary:
We are seeking a dedicated and compassionate Physical Therapist to join our Long Term Care (LTC) facility. The ideal candidate will be responsible for providing rehabilitation services to residents with physical impairments or disabilities in order to help improve their quality of life and independence.
Responsibilities and Duties:
1. Conduct thorough assessments of residents' physical abilities and limitations.
2. Develop individualized treatment plans in collaboration with the interdisciplinary team.
3. Implement therapeutic interventions to improve mobility, strength, balance, and pain management.
4. Monitor and document patients' progress and adjust treatment plans as needed.
5. Educate residents and their families on exercises and strategies for improving functional abilities.
6. Coordinate with other healthcare professionals to ensure holistic care for residents.
7. Provide guidance and supervision to Physical Therapy Assistants (PTAs) and support staff.
8. Adhere to all professional standards, ethical guidelines, and regulatory requirements.
Qualifications and Skills:
1. Graduate of an accredited Physical Therapy program.
2. Valid PT license in the state of practice.
3. Prior experience working with geriatric populations in a Long Term Care setting preferred.
4. Strong communication and interpersonal skills.
5. Ability to work independently and as part of a team.
6. Knowledge of best practices in rehabilitation and evidence-based treatment techniques.
7. CPR certification required.
8. Must be able to lift and transfer patients safely.
