The PT designs individualized therapy plans that focus on strength, balance, pain management, and daily living skills for elderly or chronically ill patients in skilled nursing or long-term residential facilities.
Evaluate residents' physical condition and functional limitations
Develop and implement customized treatment plans based on medical diagnosis and resident goals
Provide therapeutic interventions such as strength training, gait and balance exercises, and pain relief techniques
Collaborate with interdisciplinary team members, including nurses and occupational/speech therapists
Educate residents, families, and caregivers on mobility and safety techniques
Document all evaluations, treatment progress, and outcomes accurately
Comply with state and federal LTC regulations, facility policies, and best practice standards
Doctor of Physical Therapy (DPT) or equivalent degree
Current PT license in [State]
CPR/BLS certification required
Experience in geriatric rehabilitation or long-term care strongly preferred
Strong interpersonal, communication, and time management skills
