We are seeking a compassionate and skilled Home Health Physical Therapist to provide in-home therapy services to patients recovering from illness, injury, or surgery. The PT will develop individualized care plans, restore mobility, improve quality of life, and promote independence while working collaboratively with the healthcare team.
Evaluate patients’ functional status, mobility, and rehabilitation needs in a home setting.
Develop and implement individualized therapy plans to improve strength, balance, coordination, and mobility.
Provide hands-on physical therapy interventions including exercises, gait training, pain management, and functional mobility training.
Educate patients and family members on exercises, home safety, and self-care strategies.
Monitor patient progress and adjust treatment plans as necessary.
Maintain accurate clinical documentation and comply with regulatory requirements.
Collaborate with physicians, nurses, and other healthcare professionals to ensure continuity of care.
Ensure patient safety and promote independence within the home environment.
Current state licensure as a Physical Therapist (PT).
Graduate from an accredited Physical Therapy program (DPT or equivalent).
Previous home health or rehabilitation experience preferred.
Strong clinical assessment and critical thinking skills.
Excellent communication, interpersonal, and organizational skills.
Valid driver’s license and reliable transportation.
