Physical Therapist (PT) – Long Term Care (LTC)
The Physical Therapist (PT) in Long Term Care (LTC) is responsible for providing comprehensive rehabilitation services to residents with chronic conditions, post-surgical needs, and age-related mobility challenges. This role focuses on improving physical function, independence, and overall quality of life through customized therapy programs and evidence-based interventions.
The PT collaborates with interdisciplinary teams, including nursing, occupational therapy, and speech therapy, to design and implement individualized treatment plans aimed at restoring mobility, reducing pain, and enhancing patient safety.
Evaluate residents’ physical abilities and functional limitations.
Develop and implement personalized therapy treatment plans.
Provide manual therapy, therapeutic exercises, and mobility training.
Track and document progress in accordance with regulatory standards.
Educate residents and caregivers on proper body mechanics and home exercises.
Collaborate with interdisciplinary teams to coordinate holistic care.
Ensure compliance with state, federal, and facility regulations.
Maintain high standards of ethical and patient-centered care.
Bachelor’s or Doctorate Degree in Physical Therapy (DPT preferred).
Valid Physical Therapist license in the practicing state.
1–3 years of experience in a long-term care or skilled nursing facility preferred.
Strong interpersonal, clinical, and documentation skills.
Ability to work independently and as part of a multidisciplinary team.
Expertise in geriatric rehabilitation and chronic care management.
Compassionate, patient-focused approach.
Strong communication and problem-solving skills.
Knowledge of therapy documentation systems (e.g., RehabOptima, NetHealth).
Physical stamina and attention to safety.
