Occupational Therapist (OT) – Long Term Care (LTC)
We are seeking a compassionate and skilled Occupational Therapist (OT) to join our Long Term Care (LTC) team. The OT will assess, plan, and implement individualized therapy programs to help residents regain independence, improve functional abilities, and enhance their quality of life in a supportive care environment.
Evaluate residents’ functional, physical, and cognitive abilities to determine therapy needs.
Develop and implement individualized treatment plans focused on activities of daily living (ADLs).
Use therapeutic techniques to improve mobility, coordination, strength, and self-care skills.
Recommend and train residents in the use of adaptive equipment and assistive devices.
Collaborate with an interdisciplinary care team (PTs, CNAs, nurses, social workers) to optimize resident outcomes.
Document progress and treatment outcomes in accordance with facility and regulatory standards.
Educate families and caregivers on continuing care and rehabilitation techniques.
Ensure compliance with state, federal, and facility policies regarding patient care and documentation.
Bachelor’s or Master’s Degree in Occupational Therapy.
Current and valid Occupational Therapist (OT) license in applicable state.
Previous experience in Long Term Care, Skilled Nursing, or Rehabilitation preferred.
Excellent communication, problem-solving, and organizational skills.
Compassionate approach and commitment to resident-centered care.
Long Term Care (LTC) / Skilled Nursing Facility setting.
Interdisciplinary teamwork with nurses, CNAs, PTs, and social workers.
May involve lifting or assisting residents during therapy sessions.
Functional assessment and ADL training
